Below are important disclaimers and patient reminders.

Custody Cases and Other Legal Matters

Denova is not an agency that works with custody issues. Staff members will provide appropriate levels of medical and/or behavioral health care to your child and family; however, staff members are not permitted to make custody recommendations nor act as custody experts for any person or group associated with the case. Services are for treatment purposes only and will not be used for any legal matters

Minor Patients

Minor patient portals automatically expire upon the patient’s 18th birthday. If you have recently turned 18 and started your care with Denova, please reach out to establish a new adult portal.

Appointment Cancellations

If for any reason the patient is unable to keep the scheduled appointment it is the responsibility of the Patient, Parent, and/or Legal Guardian to call Denova staff, at least 24 hours in advance, to cancel. If the Patient fails to cancel their appointment at least 24 hours in advance, a $50 no show fee will be applied. This fee must be paid prior to being seen for any medical, psychiatric and/or therapy appointments. Note: All AHCCCS patients are exempt from this fee.

Discrimination or Harassment

Discrimination or harassment against any member of Denova Collaborative Health community (i.e., employee, patient, visitor) because of age, ancestry, color, disability as defined by Section 504 of the Rehabilitation Act of 1973 and the Americans with Disabilities Act, gender, gender identity and/or expression, national origin, pregnancy, race, religion, sexual orientation, veteran’s status or any other categories protected by federal or state law is prohibited and will not be tolerated, nor will any person for those reasons be excluded from the participation in or denied the benefits of any program or activity within Denova Collaborative Health.

Denova does not discriminate against any person receiving care because of his/her ability to pay for the services or because payments for those services are made under Title XVIII (Medicare) or Title XIX (Medicaid/AHCCCS), or for reasons of race, color, sex, national origin, disability or religion.

Reporting a Compliance Concern:

If you have a compliance concern please reach out to the Compliance Helpline (602) 314-5189. The Compliance Helpline is voice message ONLY and has no caller ID to provide anonymity.

All correspondence will be treated confidentially (to the extent the law will allow), reviewed, and investigated, as appropriate.
