The Medical Records Department strives to make accessing your medical records a secure and smooth process by offering several options to access your health information.

Response Timelines

While we know obtaining your records is an important task, we do have time frames in place to make sure we are following HIPAA guidelines. We thank you in advance for your patience.

  • Stat: 24 hours

  • Medical Record: 2-3 business days

  • Behavioral Records: 10-15 business days

Request for Amendment

Patients have the right under HIPAA to request Denova Collaborative Health to have their medical record amended/corrected if he/she believes that the records entries have been made in error. All requests to have patient records amended should be sent to the Compliance Department with the completed Request to Amend Form.
